These Terms and Conditions govern your use of Children's Clinic LLC DBA 5 Degree Academy services, including our website and our SMS and email communications programs. By using our services or opting into our communications, you agree to these Terms and our Privacy Policy.
Children's Clinic LLC DBA 5 Degree Academy may send SMS and email messages to users who opt in. Messages may include promotional offers, service updates, appointment confirmations, and appointment reminders. Message frequency may vary.
Our services and messaging programs are intended for individuals who are at least eighteen (18) years of age. By opting into our SMS or email communications, you represent and warrant that you are 18 years of age or older and have the authority to provide consent to receive communications from Children's Clinic LLC DBA 5 Degree Academy. We do not knowingly collect or solicit information from individuals under the age of 18 for participation in our messaging programs.
By providing your phone number and opting in, you authorize Children's Clinic LLC DBA 5 Degree Academy to send text messages to your mobile number. Consent is not a condition of purchase.
You can cancel the SMS service at any time. Just text "STOP" to the (312) 488-7178. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us.
If you want to rejoin after opting out, you can opt in again using the same method you used to enroll originally.
If you are experiencing issues with the messaging program you can reply with the keyword HELP for more assistance, or you can get help directly at [email protected] or (312) 488-7178.
As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive up to 11 transactional messages per booked program (including booking confirmations and appointment reminders). Promotional messages may be sent periodically and will not exceed 4 messages per month. If you reply to a message or contact us directly via SMS, we may respond to your inquiry; message frequency will vary based on your interaction. SMS is not monitored 24/7. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
Carriers are not liable for delayed or undelivered messages.

5-Degree Coaching Academy provides mental skills coaching and training to coaches, athletes and their parents.
Although both coaching and psychotherapy utilize knowledge of human behavior, motivation and behavioral change, and interactive counseling techniques, the major differences are in the goals, focus, and level of professional responsibility. 5-Degree Coaching Academy does not provide mental health services. Under the terms of this agreement, 5-Degreee Coaching Academy does not examine, diagnose, evaluate or treat any mental health conditions.
It is important to note that although some of our staff are Licensed Clinical Psychologists, none of their Coaching Services shall be interpreted as the provision of therapeutic, counseling or mental health services. Client acknowledges that coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling, psychotherapy, psychoanalysis, mental health care, substance abuse treatment, or other professional advice by legal, medical or other qualified professionals and that it is the Client’s exclusive responsibility to seek such independent professional guidance as needed.
We cannot guarantee any results or outcomes of the Coaching services offered by 5-Degree Coaching Academy. As with any Coaching-related program your results may vary, and will be based on many variables, abilities and other factors outside 5-Degree Coaching Academy’s control, so therefore no guarantees or warranties can be made. You agree that 5-Degree Coaching Academy and employees or affiliates are not responsible for the success or failure of your personal athletic performance, decisions, goals and relationships, or any other result or consequence of any kind that you may have as a result of information, training and resources presented to you through our Coaching services. You are solely responsible for the outcomes, consequences and results for your athletic skills, life choices, and wellbeing. There is no guarantee that Clients will reach their goals as a result of participation in our program.
Client accepts and agrees that Client is 100% responsible for its progress and results from the Coaching Services provided. 5-Degree Coaching Academy makes no representations, warranties or guarantees verbally or in writing regarding Client’s performance. Client understands that because of the nature of the Retreat and extent, the results experienced by each Client may significantly vary.

This Agreement shall be governed by and construed in accordance with the laws of the state of Illinois, regardless of the conflict of laws principles thereof.
